To bridge gaps between the Somali immigrant community and the health care system, and to improve healthy eating and activity.
Project Description
The Circles of Health & Well-Being is an eight-module, culturally based program designed to empower women by sharing evidence-based health information, resources and tools that support healthy lifestyles. The program tailors activities to individual needs, and combines support of friends a family, shared cooking experiences, culturally appropriate exercise, nutrition and health education in a safe, welcoming and healing environment.

How Health Plans Support the Project
UCare supported this program by providing grant funding for the assessment of needs in the Somali community of Faribault, Minnesota, as well as for the implementation of the Circles of Health & Well-Being program in Rochester and Faribault.
