Forms
Uniform Facility Credentialing Application. Now, instead of filling out slightly different forms, people who run facilities across Minnesota can use this common form when they apply to care for Minnesotans. Nonprofit health insurers in Minnesota all accept the same form. This video on YouTube tells you more.
Disclosure of Ownership. Providers sign and return a disclosure statement as part of contract requirements between health insurers and the Department of Human Services. This form is approved for use with Blue Plus, HealthPartners, Medica, Hennepin Health and UCare.
